Education and Training Topic Group
• Developed and implemented a training needs analysis
• Developed and implemented a comprehensive set of education packages
• Devised the “Meeting Cards”

GP Registration and Data Collection
• Agreed LD Read coding System that would meet local demand
• Implemented a Local read Coding System - with training that also identifies self-advocates as carers
• Data Collection exercise that identifies “Current Known Population” - 1700

Health Action Planning

• Agreed 4 levels to Health Action Planning
• Agreed Primary Care Services to provide Health Action Planning as a Main Stream Service
• Agreed with Local Providers, self Advocates and carers an approach of Health Action Planning that would meet everyone's needs
• Agreed a Local GP Pilot Site
• Agreed the need for accessible Communication Documentation
• Secured temporary funding for an Accessible Communications Worker
• Attended the GP Market Place Event”

Diet, Exercise and Leisure

• Research carried out into the Local levels of Obesity and under weight
• Applied for local funding to implement exercise classes
• Agreed to facilitate leisure/exercise opportunities file to Local Providers of Services
