Valuing People Objective:

As young people with learning disabilities move into adulthood, to ensure continuity of care and support for the young person and their family and to provide equality of opportunity in order to enable as many disabled young people as possible to participate in education, training or employment.

What we have done so far...
Established a transition team.
The subgroup provides a forum for the discussion of issues around Transition.
The transition protocol has been developed.
Published information leaflets.
Transition event held annually.
Completed a series of presentations within schools.
Delivered partnership training programme with Connexions and Community Learning Disability Teams



What we plan to do in the future..
Carry out assessments prior to young people reaching adult services.
Produce a transition pack to highlight the transition process.
Complete an audit of those who have experienced the transition process Highlighting the successes and failures.
Continue to raise the profile of transition
Developing Person Centred Transition Reviews across Wigan
